Field Trip

	Field Trip Permission Slips

Throughout the year we will be going on different field trips. If at any time you cannot find your child's permission slip please check here for an uploaded field trip slip, or ask me for another BEFORE the trip date. Be sure to fill in the ENTIRE slip, including your child's name, the name and date of the trip (see the calendar on our website), ALL the insurance information, and don't forget to sign the permission slip.

Please also include any fee for the trip when you return the form.
